I understand that this seminar involves direct, close-proximity exposure to live honey bee colonies in an outdoor Florida environment. I acknowledge and accept that participation in this seminar carries inherent risks, including but not limited to:

Bee stings — I understand that bee stings are possible and even likely during hands-on hive work. Stings can cause pain, swelling, allergic reactions, and in rare cases, life-threatening anaphylaxis.

Heat-related illness — I understand this is an outdoor activity in Florida, in direct sun, while wearing protective equipment. Risks include heat exhaustion, heat stroke, and dehydration.

Physical exertion — I understand this seminar requires standing for extended periods, lifting hive equipment (5–10 lbs), and moving on uneven ground.

Outdoor hazards — I understand risks associated with an outdoor agricultural environment, including uneven terrain, insects, sun exposure, and weather conditions.

Bee Sting Allergy Declaration

I have a known allergy to bee stings. I have been cleared by my physician to participate in hands-on beekeeping activities. I will carry my prescribed EpiPen on my person (not in my vehicle) at all times during the seminar.

Agreement to Safety Rules & Requirements

By initialing each item below, I confirm that I have read and will comply with the following:

I will bring all required gear (bee suit or jacket with veil, gloves, smoker, hive tool, closed-toe boots/shoes, long pants, half-calf socks, water bottle, unscented sunscreen).

I will NOT bring perfume, cologne, scented products, dark clothing under my suit, loose clothing, or pets.

I will follow all safety instructions given by the seminar instructor at all times.

I understand that failure to comply with safety rules may result in my removal from the seminar without notice.

Release of Liability & Hold Harmless Agreement

In consideration of being permitted to participate in the "Get Into the Hive" Beginner Beekeeping Seminar, I, the undersigned, voluntarily agree to the following:

I hereby release, waive, discharge, and hold harmless Mike Dunleavy, Mad Mike's Bees, Pinellas Park Agriculture Farm, and their respective officers, employees, agents, volunteers, and representatives (collectively, the "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me, or to any property belonging to me, while participating in this seminar, whether caused by the negligence of the Released Parties or otherwise.

I further agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs they may incur as a result of my participation in this seminar.

I acknowledge that I have read this waiver in its entirety, that I understand its terms, and that I am signing it voluntarily. I understand that by signing this document, I am giving up substantial legal rights, including the right to sue.

In the event of an emergency, I authorize the seminar instructor and/or emergency medical personnel to provide or arrange for medical treatment as they deem necessary. I understand that any medical costs incurred are solely my responsibility.
